Your Rights
As a health care consumer, you have the right to:
Considerate and Respectful Care
Have your individual dignity and respect by competent personnel.
Safety
Receive care in a safe setting and be free from all forms of abuse and harassment.
Fair
Free from exclusion and restraint that is not medically necessary or used as means of coercion, discipline, convenience, or retaliation by the staff.
Records Access
Obtain from the physician, complete current information concerning his diagnosis, treatment, prognosis, plan of care, and discharge plan of care in terms the patient can be reasonably expected to understand.
Refuse Treatment
Refuse treatment . to the extent permitted by law and to be informed of the medical consequences of his action.
Confidentiality and Privacy
Every consideration of his privacy concerning his own medical care program. All communications and records pertaining to his care should be treated as confidential.

Care
Reasonable continuity of care.
Choice of Service Providers
Freedom of choice in selecting a provider such as home health or medical supplies.
Explanation
Examine and receive an explanation of his bill, regardless of source of payment.
Conduct
Know what hospital rules and regulation apply to his conduct as a patient.
Advance Directive
Formulate advance directive & appoint a surrogate to make health care decisions on his/her behalf to the extent permitted by law.
Designate
Designate representative to participate in the consideration of ethical issues.
Make a Decision
Get a decision about health care payment and coverage of services.
Request
Request a review or appeal decisions about health care payment.
Treatment
To appropriate assessment and management of pain.
Communication
To be communicated with in a manner that is clear, concise, and understandable. A patient who does not speak English will have access, when possible, to an interpreter.
Non-discrimination
To medical and nursing services without discrimination based upon race, color, religion, sex, sexual preference, national origm, or source of payment.
