Our mission is to provide comprehensive assistance to healthcare workers and patients by managing an innovative health record system. We maintain privacy and confidentiality while ensuring quality care and access to vital health information.
Services We Offer
- Care, custody, and control of health records in both paper and electronic formats.
- Oversight of operational functions related to collecting, protecting, and archiving legal health records.
- Preservation of patient confidentiality and protection of their right to privacy.
- Facilitation of health information exchange among healthcare providers within our facility and elsewhere.
- Maintenance of secondary reports and indexes to provide vital statistics and research information for Tribal agencies, as well as state and federal offices.
- Management of supplemental insurance claims for Aflac.
- Compliance with the regulatory requirements of Health Insurance Portability and Accountability Act (HIPPA).

Eligibility
For individuals seeking services at Choctaw Health Center, eligibility is determined under current Indian Health Services (HIS) regulations. Eligibility criteria include:
- Persons of Indian descent
- Non-IHS eligible pregnant females with IHS eligible male partners.
- Specific documentation is required to establish eligibility, including documentation of an Indian descendant of a Federally Recognized tribe and residency.
Required Documents to Establish Eligibility
Proof of descendancy for person of any age may be established by presenting any of the following:
- Adopting decree
- Marriage license
- Custody order from Child Welfare Agency
- Guardianship Order
Plus one of the following:
- Birth Certificate
- Notarized affidavit from eligible parent attesting to parentage
- Documentation required for Non-IHS eligible mother pregnant with Indian child
- Proof of descendancy of the father of the child (Certificate of Degree of Indian Blood (CDIB), tribal enrollment card from federally recognized tribe, letter of descendancy issued by a federally recognized tribe or tribal citizenship care from federally recognized tribe)
- Proof of marriage or notarized acknowledgement of paternity
Documentation Required for Name Change
An established patient who desires to have his or her name changed on his or her medical record shall submit one of the following documents reflecting the name change:
- Marriage license
- Divorce decree
- Annulment decree
- CDIB
- Document issued by a federally recognized Indian tribe
Medical Records Request
Patients must sign a Release of Information Form to receive physical copies of their medical record. We ask that you provide the items listed below:
- Driver's License
- Or
- Tribal enrollment card / Certificate of Indian Blood (CDIB)
