Eligibility

  • Patient whose medical condition requires inpatient care will be eligible for services in the Inpatient Department.

Admission Process

1. Referral or Admission Request

Patients may be admitted through a referral from a primary care physician or can seek admission directly if they require immediate medication attention.

2. Initial Assessment

Upon arrival, a nurse or hospitalist will conduct an initial assessment to determine the medical needs and urgency for hospitalization. This may include a review of the patient’s medical history and current condition.

3. Patient Registration

Patients or their guardians will need to complete registration forms, providing personal information, insurance details, and emergency contacts. Required documentation includes driver's license, and insurance card.

4. Medical Evaluation

Following registration, medical staff will evaluate the patient’s condition. This may involve diagnostic tests, consultations, and any necessary pre-admission screenings within the hospital area.

5. Room Assignment

Patients will be assigned a bed based on their medical needs and the availability of rooms in the inpatient department. Hosptial staff will escort patients to their designated rooms.

6. Care Plan Development

Our healthcare team, including hospitalist, nurses, and specialists will create an individualized care plan tailored to the patient’s specific medical condition and treatment goals.

7. Orientation to the Unit

Upon settling in, patients will receive orientation about the unit, including information on daily routines, rights and responsibilities, and available services, such as dining options and the Good Health Network.

8. Communication with Family

Patients are encouraged to designate a family member or friend who can be informed about their care and progress. Arrangements can be made for family visitations during designated hours.

9. Beginning of Care

Once admitted, the patient’s care and treatment will commence, with ongoing support from medical and nursing staff.

This process ensures that patients receive comprehensive and immediate medical care while making them feel comfortable and informed throughout their hospital stay.

During Your Stay

  • Patients will receive personalized care plans with regular updates on their progress
  • Visiting hours are from 1:00 PM to 9:00 PM daily, designed to balance support with the need for rest.
  • Patients have the right to receive visitors whom he/she designates and the right to withdraw or deny such consent at any time. Visitors that are sick are encouraged to refrain from visiting.

Parents of Minor Patients

It is strongly recommended that a parent stay at the bedside if the patient is an infant or small child. Our staff will assist the parent with the infant, if a break is needed. All children under the age of twelve (12) must have an adult stay with him/her, while in the hospital. All children under the age of three (3) will be placed in the crib.

Good Health Network

Choctaw Health Center has partnered with Good Health Network to turn the television in your patient room into a fully interactive resource for you and your family. Through your television, you can use the Good Health Network system to:

  • Learn more about your medical condition
  • Access important hospital information

  • Make sure the TV is on
  • Turn to channel 36

Hospitalists

A hospitalist is a physician who specializes in the care of hospitalized patients. If you do not have a family physician, or your physician has arranged for hospitalists to admit and direct care for his or her hospitalized patients, the hospitalist provides medical care for you until you are discharged from the hospital.

Isolation Room

Because we care about your stay, if you are admitted in isolation room then you are able to request an Ipad from the nurse’s station to Facetime your family.

Telephone Operation

The telephone is located on the bedside table.

  • To reach a department in the facility, enter the extension you wish to reach.
  • To make a local call outside of Choctaw Health Center, press 9 then dial the number you wish to reach.

Personal Belongings

We encourage you to entrust your valuables to a friend or relative while you are in the hospital. The hospital cannot be responsible for lost items. Do not bring watches, jewelry, cash or other valuables to the hospital.

Discharge Process

Upon stabilization, a discharge planning process begins, including teaching patients about their care at home and any additional follow-up needed.

Hospital Consumer Assessment of Healthcare Providers and Systems (HCAHPS) survey

The HCAHPS survey will be sent to your home address after you are discharged. This survey will measure patient satisfaction and quality of care received during hospitalization. We value this feedback to improve our services continually.
