About
The Vision Clinic serves as an important part of the Choctaw Health Department providing services in the routine eye exams, glasses prescription, diagnosis, treatment and prevention of ocular disease such as diabetic eye disease. This treatment includes annual preventative dilated eye exams as well as emergency care for new complications.

Policy
Scheduled Appointment
All patients must schedule an appointment. First come, first serve basis only.
Late Appointment
Vision Center does not offer a grace period. You must reschedule your appointment if you are one minute late.
Walk-Ins
- Limited to patients determined by the care level needed for vision services.
- Emergency care patients are given highest priority for vision care treatment without an appointment.

CHC Eye Glass Program
The CHC Eyeglass Program through the General Health Fund funded by Choctaw Health Center.
General Health Fund (GHF)
- Must be an MBCI Tribal Member
- MBCI Tribal Members blood quantum equals or exceed more than 1/2 degree
Guidelines
- Patients must have a current eyeglass prescription within CHC Vision Center or from an outside provider.
- MBCI Tribal Members who are 18 years or younger are eligible for $125.00 toward frames and lens each year.
- MBCI Tribal Members who are diabetic and person over the age of 50 are eligible for $150.00 towards frames and lens each year.
- MBCI Tribal Members who are between the ages of 19-49 years of age are eligible for $150.00 toward frames and lens every two-years.
- If a patient has Medicaid that covers frames and lenses, that insurance will be used as the primary source of funds for materials.
- Funds from the General Health Fund are only available to be used in the CHC Vision Center unless prior authorization for reimbursement has been obtained from the Vision Center Optician. The patient will pay for their glasses and submit receipts to the Vision Center Optician for reimbursement. The Vision Center does not guarantee reimbursement for any outside materials.
Payment Policy
When placing an order for glasses we require payment in full before placing the order. You will be responsible for any out-of-pocket costs that are not being covered by CHC General Health Fund and/or other insurance coverage.
- The estimated price of the eyeglasses shall be paid in full before the eyeglasses are ordered through Primary Eyecare and Optical of Meridian. Partial payments will be accepted, but the glasses will not be ordered until it is paid in full.
- Payment is expected at the time of service. We accept cash, money order, MBCI payroll deduction form and other CHC approved Tribal Entity are accepted. We do not accept checks.
- If a patient has Medicaid or another insurance that covers frames and lenses, that insurance will be used as the primary source of funds for materials.
- MBCI Payroll Deduction Agreement Form will be accepted, but the glasses will not be ordered until it is approved by the MBCI Finance Office Loan Officer. Once a patient signs the Payroll Deduction Agreement Form, the patient will abide by the loan terms listed on the form and during the duration of the Payroll Deduction Process the patient does not have the right to halt or cancel anytime without prior approval from the Vision Center's Optician.
