Choctaw Health Center offers a variety of prosthetic services with the goal to promote the health, independence and quality of life for our patients. In order to provide this specialized service, Choctaw Health Center aligns with local certified prosthetic service providers. Patients with prosthesis have lifelong needs in the area of prosthetics and equipment.
Some of the needs that Choctaw Health Center may cover include:
- Legs
- Arms
- Breasts
- Eyes
- Socks
- Shrinkers
- Bras
- Initial prosthetic prescription fabrication and fitting
- Routine repairs and replacement
- Need for new prosthesis as functional abilities change
- Orthotics may be covered- prescription by a Physician
Eligibility
- Enrolled MBCI Tribal Member
- Patient's responsibility to get enrolled member information on their patient face sheet before supplies can be obtained
- All prosthetics must have prescription from a physician/nurse practitioner, as well as needed paperwork for insurance.
Patient Responsibility
- Patients are expected to take care of their prosthetic. Any repairs or replacement for any misuse/abuse/neglect will be the patient's responsibility.
- Managed Care follows insurance guidelines in regard to prosthetics. If patient is not compliant, per insurance guidelines, patient can pay out of pocket.
- If insurance requires any initial or follow up physician visits patients must comply with seeing physician or the prosthetic will not be provided.
