About
Physical therapists are trained and licensed movement experts. They can diagnose and treat a range of injuries, disabilities, and health conditions. Their objective is to improve a patient's range of movement and quality of life to prevent further iqjury or disability. All patients are required to have a referral from a provider from Choctaw Health Center or any other outside provider to receive services.

Appointment Policy
You may bring in your doctor order or have them to send it to our Physical Therapy Department. Once we receive the order we will schedule you an appointment for our services.
Cancel Appointment
- Patient must notify the Physical Therapy Department 24 hours in advance if they are unable to keep their appointment.
- Appointment can be rescheduled for a later date.
Late Appointment
In order for us to provide timely, attentive service to everyone, you are expected to be on time for scheduled appointments.
- ff you are more than 15 minutes late, you must reschedule your appointment.
Failed Appointment
- Patient considered a no call no show for scheduled appointment after second missed appointment will not be seen.
